Older Adult Access System of Care

The Older Adult Access System of Care serves seniors who are struggling with mental illness. The program reaches out through community organizations such as senior centers, peer centers, referrals from APS, law enforcement, and local community clinics. We travel to referred people's homes, meet them in office, go to the senior centers, attend multiple agency meetings to discuss clients being referred for services or current clients that have APS referrals.

This program linkage and access to needed services such as individual/group therapy and individual/group rehabilitation services.
